No ID card needed — just supply your name, date of birth and social security number (or your member number, which you’ll find on your ID card after logging in). If you’re a dependent, you’ll need these details for the primary member.
Check your electronic statements to see what, if anything, you owe the dentist. You’ll also be able to browse previous statements and download them for your records.
