If you are a health care professional who provides care to special needs plan (SNP) members contractually or routinely, the Centers for Medicare & Medicaid Services (CMS) requires you to complete initial and annual model of care (MOC) training. This requirement includes delegated medical groups.
As a company that offers Special Needs Plans (SNPs), we’re required by CMS to help you meet this requirement by providing this training to you. We offer our SNP MOC training as a 10-minute, self-paced course.
Important points
- There is no national MOC training because care management processes vary across insurers. This means you may be asked to complete multiple trainings as required by other insurers.
- The deadline for training completion is Dec. 31
- We require compliance with provider education and training according to CMS requirements
- You don’t have to take the training if you only see UnitedHealthcare commercial members
- CMS may request an attestation of the initial and annual training
