Instructions
The Montana End-of-Life Registry service provides registered health care providers and consumers with access to stored advance health care directives. There is no cost to use this service.

General Tips
When using the Registry service to search for an advance directive, keep the following in mind:
- Carefully check the spelling of names and sequence of numbers and other search criteria.
- When entering a name from a consumer’s wallet card, enter the name precisely as it appears on the wallet card.
- When registering an advance directive, consumers choose between two levels of privacy, which limit who may view a copy of their advance directive.
- any health care provider registered with the Montana End-of-Life Registry
- anyone with the registrant’s name and access code
- anyone with the registrant’s last four digits of his or her Social Security Number, date of birth and mother’s maiden name
- any health care provider registered with the Montana End-of-Life Registry
- anyone with the registrant’s name and access code
Adobe Reader Required to View Advance Directives
Advance directives are saved in the portable document format (PDF), and Adobe Reader is required to view PDF files. For help with viewing or printing PDF files, go to the Department of Justice Help page.
Using the Registry Service – For Consumers
People who have filed an advance directive with the Montana End-of-Life Registry can access and view the directive.
- In the Registrants box, click Start.
- Enter:
- the appropriate name and access code listed on the wallet card or
- last four digits of the Social Security Number, date of birth and mother’s maiden name
- Click View.
- If an advance directive is on file, a PDF copy of it will appear on the screen.
- To print the directive, use your browser’s Print button.
- To return to the registrant information entry page, use your browser’s Back button.
- To exit, close the window.
Health care providers who are registered with the Montana End-of-Life Registry can search for and view advance directives.
If you know the patient's name and access code from his or her wallet card:
- In the Health Care Providers box, click Start.
- Enter the patient’s name precisely as it appears on his or her wallet card.
- Enter the patient’s access code.
- Click View.
- If an advance directive is on file, a PDF copy of it will appear on the screen.
- To print the directive, use your browser’s Print button.
- To return to the search page, use your browser’s Back button.
- To exit, close the window.
If you need to search the database of registered advance directives:
- In the Health Care Providers box, click Start.
- Enter your health care provider user name/ID and password.
- Click Login.
- Enter search criteria.
- At least two search fields must be entered.
- Any combination of the search fields may be used.
- The more specific the search, the narrower the results.
- In some cases, a search using fewer fields may produce more complete results.
- Click Search.
- If an advance directive is on file, a PDF copy of it will appear on the screen. If more than one advance directive matches the search criteria, a list will appear on the screen. Click on the appropriate name to view a PDF copy of the directive.
- To print the directive, use your browser’s Print button.
- To return to the search page, use your browser’s Back button.
- To exit, close the window.
